Programs:

At the Core Care Family Health Team, family doctors work with other health care professionals such as Dietitians, Mental Health Counsellors, Nurse Practitioners, Pharmacists, Physician Assistants, Registered Nurses and Registered Practical Nurses to see more patients and to keep them healthy.

These Allied Health Professionals are clinicians who work together in a collaborative environment using various tools and resources to get patients the care they need at the time they need it.

Dietitians

The Hamilton Family Health Team has a number of Registered Dietitians who will help patients manage their nutrition needs and other health problems.

Click here for more information about Dietitians.

Mental Health Counsellors

Mental Health Counsellors have nursing, social work or other professional backgrounds. They are able to see patients with a broad range of mental health-related difficulties. In collaboration with the family doctor, the patient and counsellor will assess the problem and develop a treatment plan.

Click here for more information about Mental Health Counsellors.

Pharmacists

Clinical pharmacists with the HFHT work in the family doctors’ offices. They work directly with the family doctor and other members of the health care team.

Click here for more information about Pharmacists.

Registered Nurses (RNs) / Registered Practical Nurses (RPNs)

Registered Nurses and Registered Practical Nurses within the Hamilton Family Health Team work with our doctors, mental health counsellors, Registered Dietitians, and pharmacists to provide care that promotes health and prevents disease.

Click here for more information about Registered Nurses and Registered Practical Nurses.

Physician Assistants

The role of the PA to function as physician extenders, complementing the existing services and improving patient access to health care.

Click here for more information about Physician Assistants.
